How to register a death

To register a death the following steps must take place:

• An informant (usually a family member) and funeral director must complete the Statement of Death with information about deceased
• The Medical Certificate of Death and Statement of Death are submitted to the Municipal Division Registrar or Deputy  by the funeral director

It may take up to 12 weeks for a death to be registered in Ontario.

How to get a death certificate

There are some situations where you may need an official death certificate from the Province of Ontario. You will need to submit a Request for Death Certificate:
